The toll of Canada’s ER Closure Crisis

Canadians living in larger cities are familiar with the problem of emergency department overcrowding, where people waiting for beds languish on gurneys in hospital corridors – or what’s become known as “hallway medicine.”

 

But in small-town Canada, there is another problem, one that has been dubbed “highway medicine.”

 

Hi, my name is Tu Thanh Ha and since the pandemic began I have often written about issues stemming from personnel shortages in Canada’s public health care system.

 

Last April, a headline on the website of the Newfoundland radio station VOCM caught my attention: “‘Fortunate’ outcome despite locked emergency room in Bonavista.”

 

It was just a four-paragraph item. There was no name, just the tale of a man with a hand badly injured by a power saw. He went to the emergency room in the small town of Bonavista, only to find the doors locked. Fortunately for him, he found a nurse who got him transported by ambulance to another ER for surgery.

 

It so happens that for the past few months, I had been working with two colleagues, health reporter Kelly Grant and data editor Yang Sun, on a deep dive into the issue of hospital emergency service disruptions – night-time closures, weekend closures, shortages of doctors and nurses.

 

The incident in Bonavista, N.L., was a concrete example of the impact of such periodic closures. A local resident wasn’t aware on the day a chainsaw ripped through his hand that his local ER would be shuttered for 48 hours because of staffing issues.

 

Kelly was able to identify the man, Derrick Maloney, and interviewed him during a reporting trip to Bonavista. Maloney, and a photo of him and his bandaged hand, are featured in our in-depth feature article on ER closures.

We worked for seven months on this story as part of our “Secret Canada: Your Health” series.

 

One of the big problems with emergency service disruptions is that there is no systematic, nationwide tracking of these incidents.

 

We set out to compile a dataset of ER closures since 2019, the last year before the pandemic. This meant contacting dozens of health ministries and health authorities across the country. Most told us that we would have to submit access-to-information requests, which can be slow going.

 

It was a painstaking process but the nature of journalism is often that long, mind-numbing work is required to shed insight into complex issues.

 

In the end, only New Brunswick and Manitoba were able to provide a detailed set of closure statistics. Many provinces didn’t begin tracking closures until after the pandemic started.

 

The data we collected show that this is almost exclusively a rural problem that touches all provinces, but particularly Manitoba, with 70 per cent of ERs having experienced closures, Nova Scotia (66 per cent), and Saskatchewan (57 per cent).

 

We found that rural hospitals in various provinces tried to deal in various ways with their ER staffing problems. Some never closed but advised the population that they would only treat the most urgent cases. Others would only take walk-in patients. Certain places switched to office hours, shutting down at night and during weekends.

 

The reality was that in many small communities, dealing with a pressing medical problem meant driving to another town because the local ER was closed – “highway medicine.”

But we were able to conclude with a positive development.

 

In Rivière-Rouge, 170 kilometres northeast of Montreal, local residents rallied against the scheduled nighttime closure of their ER, went to court, got an injunction, and are now working hand-in-hand with hospital administrators to recruit and retain workers and keep medical services running.

Kiev Ablaze:

Kiev ablaze after reported overnight missile and drone barrage

Ukrainian officials claim there were at least 13 impacts, with blurred-out footage showing large fires lighting up the skyline.

 

Ukrainian officials have claimed that the Russian military launched a wave of missile and drone strikes targeting the capital early Friday. The Russian Ministry of Defense has yet to comment.


The first strikes were reported shortly after midnight, with Mayor Vitaly Klitschko urging residents to seek shelter as the capital’s air defenses engaged incoming targets. Officials confirmed that by 4am, several mostly non-residential facilities in multiple districts had been hit in successive waves.

 

There were no immediate reports of fatalities, but Klitschko said at least 14 people were injured after falling debris from intercepted missiles and drones triggered fires in residential areas and private yards.

Multiple blurred-out videos shared on Ukrainian social media appear to show the aftermath of the strikes, with numerous fires lighting up the skyline of Kiev. 


Officials confirmed at least 13 impacts in different parts of the capital.

The exact locations and types of facilities hit are difficult to verify, as the Ukrainian authorities heavily censor the flow of information and punish those who share footage of the impacts – unless a civilian facility is affected.

The Russian Defense Ministry has not yet issued a statement on the incident. Moscow regularly conducts drone and missile strikes on Ukrainian military infrastructure, maintaining that its operations do not target civilians and accusing Kiev of placing air defense systems in densely populated areas.

 

The reported strikes came just hours after President Vladimir Putin held a phone conversation with his US counterpart, during which Donald Trump reportedly urged Russia to cease hostilities in Ukraine as soon as possible. Putin reaffirmed Moscow’s openness to a negotiated solution, but reportedly stated that Russia would not back down until the “well-known root causes” of the conflict are addressed.


 

Despite several rounds of direct negotiations in Istanbul in recent months, Kiev has continued long-range attacks on Russian territory, including nightly drone raids. On Friday Morning, a woman was killed when a drone crashed into an apartment building in Rostov Region. On Tuesday, a Ukrainian drone strike caused multiple fatalities and serious injuries at an industrial plant in the city of Izhevsk around 1,000km (620 miles) east of Moscow. On Monday, a woman was killed and three others injured in a missile strike on the Donetsk People’s Republic.

A prominent Russian TV presenter said that Russia's invasion of Ukraine is approaching a "new stage" in which Moscow will find itself at war with the North Atlantic Treaty Organization — and by extension, the entire world.

 

"I believe the special military operation is entering a new stage. Ukrainians alone are no longer enough," said Vladimir Solovyov, according to the translation of a video clip tweeted on Thursday by The Daily Beast's Julia Davis.

Russian state TV propagandist Vladimir Solovyov has warned that Russian President Vladimir Putin may soon order strikes on European NATO members as Moscow's war with Ukraine continues.

The Russia-Ukraine war has recently escalated following President Joe Biden's decision authorizing Kyiv to use U.S.-made weapons to strike targets inside Russia. Ukraine used U.S. ATACMS missiles to strike Russian targets including an ammunition depot for the first time last week.


Solovyov suggested during a recent broadcast that Russia's retaliatory attack days later on a Ukrainian missiles factory, which was carried out using experimental hypersonic ballistic missiles could soon be followed up with strikes on similar targets in NATO countries.


While pointing to German arms manufacturer Rheinmetall as a potential target, Solovyov warned that Russian attacks in NATO countries would come without any warnings to civilians, since the countries do not include "our civilians."

"You didn't like the strike on [the Ukrainian missile factory]?" Solovyov said, according to a translated clip shared by Ukrainian internal affairs adviser Anton Gerashchenko on X, formerly Twitter. "Do you realize that Rheinmetall could be next? [Putin] said that we will only warn civilians in Ukraine."

 

"We don't need to warn those [in Europe]," he added. "There aren't our civilians there. That means that something could happen to the NATO bases in Poland, Romania, Britain or anywhere from where those missiles that the Americans Ukraine at targets on Russian territory are supplied."


Newsweek reached out for comment to NATO via email on Monday night.

Solovyov followed up his warning of Russian attacks on NATO countries by bizarrely tying the threat to a Bible verse that suggests gay men who have sex have "committed an abomination" and "shall surely be put to death."

 

"This brings to mind the biblical 'they shall be put to death, their blood be upon them,'" said Solovyov. "That was said in reference to homosexuals, which has some relevance to the people who make various decisions in the West. Come to your senses a little bit, all of you."

 

Putin's press secretary Dmitry Peskov accused the Biden administration of undermining "a peaceful path" in Europe favored by incoming President-elect Donald Trump in a Russian TV broadcast over the weekend, according to German press agency DPA."During the election campaign, Trump talked about wanting to make peace in some way and was leading everyone down a peaceful path," Peskov reportedly said.


"And now they are doing everything they can to escalate the situation in such a way that peace agreements are doomed to failure."
